Dr. Salar Kohan is a board-certified internal medicine physician with more than five years of experience as a hospitalist. He currently practices at Providence Cedars-Sinai Tarzana Medical Center, caring for adults with a wide range of acute and complex medical conditions.
Through this experience, he recognized how difficult it can be for patients to receive consistent physician access, clear communication, and reliable follow-up. He created MyElevateCare to offer a more personal approach to primary care—one centered on continuity, convenience, and a physician who takes the time to understand each patient’s health history, priorities, and long-term goals.
